Background Check

If you have already been fingerprinted as a volunteer coach you will only need to fill out our annual background check form. These forms must be picked up at the Recreation office, and may not be emailed or faxed.

Fingerprinting

If you have not been fingerprinted, please print out the Fingerprinting Form (PDF). In box number 7, put B08002, this is our VRN number. This is extremely important. You must enter it exactly as it appears here, no spaces, no dashes, capital B. It is the number 0 not the letter
O. If you do not fill out box number 7 properly, we will not receive your results and you will need to re-do the fingerprinting. 

You will need to fill out the form and bring it with you to your fingerprinting appointment. You can make an appointment online or by phone, the details are at the bottom of the form. The closest Sagem Morpho location in Paramus is on Forest Avenue. When you attend your appointment, please be sure to bring a valid government issued photo identification or you will be turned away. If you do not attend your scheduled appointment, do not bring a filled out form or bring proper identification, you will be charged an $11 fee on top of the $26 fingerprinting fee.

Fees

The fingerprinting will cost approximately $25 since you are a volunteer and not an employee. This cost will be reimbursed by the Recreation Commission. In order to be reimbursed, you must bring a copy of your fingerprinting form and the receipt to the Recreation office after your appointment so copies can be made and added to your file. Reimbursements will not be processed without a receipt. Any fees incurred for missing an appointment or failing to have proper documentation will not be reimbursed. A purchase order will be created by our Finance Department and will need to be signed and returned so we can process a check.
